My Medical Invoices agrees to furnish services to you as a customer, subject to the following Terms and Conditions.

Healthcare Provider Service Rates
Healthcare Providers as a customer acknowledge that the nature of the services furnished and the initial rates and charges have been communicated to you. You are aware that My Medical Invoices may change the specified rates and charges in the future provided a 30-day notice is given. The current rates and charges communicated are based on the current cost of providing services for My Medical Invoices, which these costs could change in the future.

Refund and Disputes
All fees paid to My Medical Invoices are non-refundable. This includes all payment processing fees, or any other payments made to My Medical Invoices regardless of usage or type of service. All overcharges or billing disputes must be reported within 30 days of the time the dispute occurred.

Account Cancellation
All requests for canceling accounts must be made in writing with at least 30 days' notice, sent to My Medical Invoices, Attn: Cancellations, 4606 FM 1960 West, Suite 400 Houston, TX 77069. You must include all account information to cancel.

Liability Of My Medical Invoices
You as a customer of services from My Medical Invoices acknowledge that the services provided are of such a nature that service can be interrupted or data security compromised for many reasons other than the negligence of My Medical Invoices and that damages resulting are difficult to ascertain. Therefore, you agree that the My Medical Invoices shall not be liable for any damages arising from such causes beyond the direct and exclusive control of My Medical Invoices. You further acknowledge that My Medical Invoices' liability for its own negligence may not in any event exceed an amount equivalent to charges payable by customer for services during the period damages occurred. In no event shall My Medical Invoices be liable for any special or consequential damages, loss or injury.

Jurisdiction
You as a customer acknowledge and agree that all disputes regarding My Medical Invoices shall be resolved by litigation in the courts of the State of Texas.

Refusal of Service
My Medical Invoices reserves the right to refuse, cancel or suspend services to any client for any reason at its sole discretion.

Warranty
THE MY MEDICAL INVOICES SERVICE IS PROVIDED AS-IS WITH NO WARRANTY, INCLUDING AND WITHOUT LIMITATION, ANY WARRANTY THAT OUR SERVICE WILL BE UNINTERRUPTED, ERROR FREE, WITHOUT DELAY, IMPLIED WARRANTIES OF MERCHANTABILITY OR FITNESS FOR A PARTICULAR PURPOSE. WHILE MY MEDICAL INVOICES USES BEST PRACTICES TO SECURE OUR SERVICE AND CUSTOMER DATA, MY MEDICAL INVOICES DOES NOT GUARANTY THAT OUR SERVICE OR CUSTOMER DATA CANNOT BE COMPROMISED.

REVISIONS TO TERMS AND CONDITIONS
My Medical Invoices reserves the right to revise, amend, or modify this Terms and Conditions and our other policies and agreements at any time and in any manner with or without notice.
